Information we collect
We collect only the contact and request details shown on the appointment form. Do not send medical histories, Social Security numbers, insurance member or policy IDs, card details, payment information, or other sensitive information.
How information is used
Submitted information is used only to respond to an appointment request. The website does not create a patient portal, medical record, or insurance eligibility determination.
Delivery and storage
Appointment requests are stored in the protected owner dashboard through the configured Upstash service and are also transmitted through the configured email provider. The dashboard adds an operational status, delivery status, expiration date, and an optional limited owner note; authorized staff may export the visible fields to a private CSV file. Form values are not saved in browser storage. Stored dashboard records and mailbox messages use a provisional 90-day retention target that the business must approve and configure before launch.
Analytics and cookies
Analytics are disabled unless the business configures Plausible. Permitted events contain no form values or personal details. The default site does not use nonessential cookies, so it does not display an unnecessary cookie banner.
Your choices
You may call the office instead of using a form. Contact the office to ask about a submitted request or request deletion from the protected dashboard and business mailbox, subject to applicable requirements.
Future health information
The website does not claim HIPAA compliance. Any future collection of protected health information requires a separate legal, security, and compliance review.
